About the Upstate Medical University Nurse Practitioner Residency and Fellowship (UMUNPR) Program

Founded in 1834, UHCC has a longstanding history as the region's leading healthcare organization. UHCC is located in the geographic center of NYS, serving 1.8 million people, often the most seriously ill and injured at three hospitals including the region's level I trauma center and a pediatric-focused hospital, a dedicated cancer center, and numerous satellite sites. UHCC is accredited by DNV. UMU's catchment area stretches from the Pennsylvania border to Canada, covering the entire CNY region, much of which is predominantly rural and designated as HPSA areas. Clinical care, too, is spread far and wide, through the meaningful use of technology and online support to the entire state through the Upstate Poison Center. As a biomedical research enterprise, UHCC focuses on the most prevalent human diseases including cancer, diabetes, heart disease, nervous system disorders, vision, and infectious diseases. The population of the region has significant unmet needs for PC and behavioral/ maternal health care services. Because of the varied populations in the catchment area, there are diverse challenges in providing needed services with full consideration of multiple social determinants of health. This provides a unique opportunity for UMUNPR residents to work with identified underserved populations in community-based settings.

East Hill Medical Center has served Cayuga County residents for over 50 years and is recognized by the federal government as a Federally Qualified Health Center (FQHC). As an FQHC, East Hill’s services are available to all persons regardless of age, marital status, or income level, and are always completely confidential. EHMC offers sliding scale fees to ensure no one is denied healthcare due to inability to pay. EHMC is committed to high service standards, professional ethics, and the protection of human rights. EHMC offers integrated primary care from birth to end of life including behavioral health, dental, adult, pediatric, and family medicine. EHMC’s umbrella encompasses Duckett Family Medicine serving clients aged 16 and older, Adult Medicine focused on acute care, Summit Pediatrics serving children from birth to age 21, East Hill Dental, and the growing behavioral health unit Inner Wellness which is infused into all practices. Staff work in community-based settings as well to administer COVID-19 vaccinations and boosters and provide school-based dental care. EHMC’s highly professional and diversified staff works collaboratively as a team, often referring patients internally between offices to provide comprehensive, integrated health services. EHMC offers routine medical care, vaccinations, chronic disease management, preventive healthcare, school and work physicals, well-child and sick visits, developmental screenings, dental screenings, cleanings, and treatment, reproductive healthcare, nutritional consultations, support for breastfeeding mothers, ADD/ADHD and behavioral screenings, addiction medicine, psychiatric evaluations, crisis support, therapy, care coordination, and more.

Liberty Resources, Inc (BCLR) is a 501(c)(3) not-for-profit corporation, established in 1978 and headquartered in Syracuse NY. BCLR’s mission is to assist individuals and families in need of achieving improved quality of life by providing Health and Behavioral Health services tailored to meet their particular needs. LRI provides a broad range of integrated Behavioral Health, Physical Health, and Social Services. BCLR has expertise in providing innovative services to children/youth/adults with complex behavioral health disorders/chronic medical conditions. BCLR has consistently provided culturally sensitive services across various outpatient clinics, recovery centers, employment, mobile crisis, crisis respite, foster care, community residences, supportive living/housing programs, and within our Health Home Care Management programs. Our person-centered, evidence-based services effectively lead to successful outcomes in children and adults with chronic behavioral health/medical/addiction co-occurring conditions.

BCLR serves over 5,000 individuals annually for Behavioral Health services including individual, family, and group psychotherapy for children, adolescents, adults, and families. Clients engaged in psychotherapy with an identified need are also eligible for psychiatric evaluation, medication management (including injectable medication), and case management. Medication Assisted Treatment (MAT) for substance use disorder is also offered, as well as crisis services and peer support. BCLR serves 2,000 patients in primary care under DOH offering adult, pediatric, and reproductive health services. BCLR has an on-site, integrated pharmacy through Genoa Healthcare as well as onsite laboratory services. BCLR was granted a SAMHSA CCBHC grant in 2020 and is a current CCBHC-IA grantee. BCLR is also NCQA PCMH recognized. Approximately 73% of BCLR’s clients are enrolled in Medicaid/Medicaid Managed Care, 17% are enrolled in commercial insurance and 10% are enrolled in Medicare.

The Onondaga Nation is a member of the Haudenosaunee Confederacy. The Haudenosaunee is translated to the People of the Longhouse which is an alliance of native nations united for hundreds of years by law, traditions, beliefs, and cultural values. The Haudenosaunee is also referred to as the Iroquois or the Six Nation Confederacy. The Haudenosaunee consists of the Mohawk, Oneida, Onondaga, Cayuga, Seneca, and Tuscarora nations. The Onondaga Nation lies in the middle of the Haudenosaunee territory and is also known as the Central Fire. The Central Fire is a term used to describe Onondaga as a place where the Hoyá•neh (Chiefs) meet in a Grand Council. Because of this, the Onondaga Nation is also referred to as Ganakdagweñni•yo’geh or the Capital. Onondaga is still governed by our traditional government. Making Onondaga one of the oldest continuous democratic governments on Turtle Island. The tenants of the Great Law have kept the fourteen Onondaga Chiefs and their Clan Mothers caretakers of our land, ceremonies, and governmental relations with other nations. Because Onondaga has resisted all attempts by the United States to change Onondaga into a “tribal elective system”, the Onondaga Nation continues to maintain its sovereignty.
